图2:2021 ESICM-2021年国际脓毒症及脓毒性休克管理指南:拯救脓毒症运动Icne教授[6]提出了一种基于微循环的容量管理优化模型。这种基于使用手持显微镜优化微循环携氧能力的模型,可能为每位患者得到最佳液体量(方案)提供生理学基础支持,微循环液体反应标志了成功的液体治疗。Ashish K Khanna及Kunal Karamchandani教授则提出了大循环与微循环相结合的治疗策略[10](图3)。由于微循环的评估技术并未普及,目前并未找到治疗微循环的最佳方案,合理的方案是先解决大循环相关的指标。脓毒症休克患者如果出现器官功能障碍,先测量大循环的情况并进行治疗,早期复苏的措施也会在一定程度上改善微循环,如果患者临床上出现改善则不需要进行微循环监测或干预。然而如果器官功能进一步恶化,则可能意味着患者出现了血流动力学一致性的失协调,此时针对大循环的干预可能会影响微循环,因此监测微循环并适时对微循环进行干预是合理的。
